Investigation Summary

Employee's fingertip amputated in rotary cutter

At approximately 5:00 p.m. on January 29, 1996, Employee #1 was using a Olefin rotary cutter, which is a hand tool with a 2 in. diameter circular knife. Olefin makes a guard to protect against cuts when it is not in use and provides a cutting mat and a 1/8 in. thick by 6 in. wide by 24 in. long plastic cutting edge for a guide. All these items were in use at the time of the accident. Employee #1 was cutting four layers of knit cloth while holding the plastic guide in her left hand. The knife went up and over the plastic guide, amputating a portion of the tip of her index finger, approximately 1/2 in. down from the top and 1/4 in. in from the side. There was no bone loss and the tip was sewn back on at the hospital.
