Employee fractures foot when run over by moving forklift

At or about 4:35 p.m. on December 23, 1993, Employee #1 was attempting to get on an empty pallet on the forks of a moving Nissan forklift model #331 when the forklift was making a left turn in front of the receiving office of the warehouse. The forklift hit Employee #1, knocking him off balance. He fell and his left foot was run over by the forklift wheel. He suffered a serious fracture of his foot. He was admitted in Chino Valley Medical Hospital and was released on December 28, 1993. Employee #1 was an experienced warehouse worker and knew forklift rules and regulations. He worked for the company for more than 12 years. Employee #1 stated that the accident occurred due to his own mistake.
