Employee #1 died of asphyxia from silica inhalation

Employee #1 worked in all areas of the plant, except the mold and pour area were the duties included mold set up and mold mixing. The operation involves the use of silica sand 75 percent mixture and engineering controls are not present. Approximately five years ago respiratory protection was not mandatory and respirator use was at the discretion of the employee(s). The employer manufactures bronze cemetery memorials and burial and mausoleum urns. The Employer utilizes bronze alloy 2 percent lead, 89 percent copper, and 5 percent tin and silica mixture (a-quartz) in the production process. Overexposes to copper and irrespirable silica was documented. Copper exposure measured at 158 milligrams per cubic meter. Silica (irrespirable) exposures measured at 0.13 and 0.24 milligrams per cubic meter. The higher level was measured at the work station where decedent once worked. Two additional employees with pulmonary abnormalities were identified and not listed.
