Employee's eye injured by exploding prank pen

At 6:45 a.m. on January 2, 1994, Employee #1 suffered minor eye strain when a prank pen exploded. A friend of the employee had put a prank pen in her desk drawer; when Employee #1 opened the pen, it exploded. Employee #1 went home for the day. No violation was observed.
