Employee killed when crushed by flange

At about 1:30 p.m. on March 22, 1995, Employee #1 was rolling a large wooden flange on end into the shop from the yard so that it could be put into a rack and assembled into a reel. The flange, which was 90 in. diameter and weighed approximately 350 lb, fell over and landed on Employee #1's head, killing him. No equipment was being used to prevent the wheel from falling over. The branch manager stated that the flanges fall over about 25 percent of the time and the employees are told to make sure there is room for them to run away if the flange falls.
