Employee's Finger Amputated by Wire Extruder

Employee #1 was tending the take-up reels on a wire extruder. One reel was full and the employee was preparing for a changeover, at which point the wire would switch over to a second, empty reel and continue winding while he took the first roll off. When the changeover began, the wire began to load onto the empty reel; however, when Employee #1 attempted to cut the wire between the two reels with an automatic knife, the knife failed. The employee then either attempted to cut the wire with a pair of wire cutters he was carrying, or caught his hand in a loop of wire that looped out from the drum. Employee #1's hand was caught between the wire and the rim of the reel, and his left middle finger was amputated.
