Event: 03/16/2001
Two employees sustained burns when residual gas ignited

At 5:50 p.m. on March 16, 2001, Employee #1 was installing a tee connection for the additional new gas stove in the kitchen area of the "R" Ranch Market Number 6. While Employee #1 was removing the old gas valve, he noticed that the system was still pressurized although he already turned off the main gas valve outside of the building. A coworker, a security guard, was called in by Employee #1 to assist him by covering the leak with a rag. Employee #1 screwed the pre-made tee connection on in order to control the gas leak by plugging two holes on the tee connection, but the gas leak was uncontrollable. Then, all of a sudden, the gas ignited and burnt Employee #1's face and coworker's arm. Employee #1 and coworker were transported to Harbor UCLA Medical Center for emergency care. Employee #1 was hospitalized. The Division determined that the contributing cause of the accident was that Employee #1 did not correctly turn off the main gas pipe, bled the residual gas in the pipe, and rechecked to confirm that all pressurized gas in the pipe had been discharged. Employee #1 performed a lockout and tagout on the main gas valve prior to doing the assigned task.
