Employee struck when conveyor falls off forklift

Employee #1 and two coworkers were attempting to move a conveyor with a forklift. The forklift operator failed either to center the load on the forks or to secure the load to the forks prior to moving the conveyor. When the operator moved the forklift, the conveyor tipped over, striking Employee #1 and knocking him to the floor as he tried to run away. He did not suffer a serious injury.
