Employee's finger smashed in perforating machine

Employee #1, of Aire-Pak Materials, was operating a perforator machine that ran bubble wrap film through a toothed blade perforator head. The head is set to come down and made a perforation every 12 in. so the bubble wrap can easily be torn off. The roll of bubble wrap had ended on the unwind stand, so Employee #1 was holding the tail end of it, guiding it into the machine and keeping it taut. The film on the roll was dry and sticky and when it went through the in-running draw rollers, Employee #1 was unable to release his left hand. It was also pulled into the roller and his index finger was crushed and broken. The machine had to be dismantled in order to free his hand.
