Employee's fingertips amputated when caught in nip point

A coworker told Employee #1, a maintenance electrician, that he heard a noise coming from the drive motor of the paper dust collector. Employee #1 climbed up to where the motor was located, out of sight of the floor area. He shut off the power panel and locked it out, as trained. He adjusted the dual belts, removed the lock, and ran the unit. Employee #1 shut the power off but did not reattach the lock. He reached into the machine with his gloved right hand to test the tension before the belts had come to a complete stop. His right hand was pulled into the nip point between the belts and pulley and the tips of his third and fourth fingers were severed to the first joint. He was able to climb down for help and was taken by ambulance to Kaiser Hospital in Walnut Creek, where he was treated and released. No employer knowledge could be substantiated.
