Investigation Summary
At approximately 7:21 p.m. on August 4, 2005, an employee was nailing roof sheathing along the leading edge using a pneumatic nail gun. The employee was not wearing fall protection equipment. The structure had no protective fall systems in place. The employee fell 26-ft to the ground level and was killed.
