Employee Amputates Finger During Forklift Operation

At approximately 8:00 a.m. on January 26, 2005, Employee #1, a truck driver, was assisting a forklift operator to lower a wooden pallet containing sheet metal parts. They were moving the metal parts from the bed of Employee #1's box truck to the ground. As the forklift, Yale-type, was lowering one of the pallets, the forklift operator asked Employee #1 to stand on the load to provide balance. Employee #1 stood on the pallet and rested his right hand on the mast. The load was then completely lowered to the ground and one of Employee #1's fingers on his right hand was crushed. His finger became caught between the chain drive and the mast, causing it to be amputated.
