Employee injured while operating a forklift

Employee #1 was backing up the forklift when he lost control and the forklift overturned. He sustained a fracture of his foot and he was hospitalized. There were many factors that may have contributed to the accident. For example, the loose dirt, slight incline, reckless driving, elevated mast, and wheels being turned while backings up. However, the cause of the accident was not determined.
