Employee killed in fall from ladder

Employee #1 was using a 14 ft extension ladder to install cellulose insulation in the exterior walls of a two-story house. A gust of wind blew over a second, unattended ladder, which struck Employee #1's ladder. He fell approximately 10 ft onto the concrete driveway, landing on the side of his head. Employee #1 was killed. He had not been using head protection.
