Piece of conveyor fell on him

Employee #1 and coworkers were using a forklift to move a 30 ft long section of roller conveyor. They had centered the section on the forklift, but not secured it. As the forks were being raised, the section rolled forward and fell off, striking and crushing Employee #1's chest. He was pronounced dead on arrival at Camden Clark Hospital in Parkersburg, WV.
