Employee run over and killed by backing asphalt truck

At about 4:45 p.m. on October 1, 1991, Employee #1, a flagman for a road paving crew, directed the last truck of the day into position to dump its load into an asphalt paver machine. While the asphalt's temperature was being taken, the employee went behind the truck, out of the driver's view, and leaned against it. As the employee started to walk away, the driver put the truck in reverse and the back-up beeper sounded. Employee #1 did not hear the beeper and the truck backed over him. He was killed.
