Employee suffers contusion when struck by falling pallet

At approximately 7:35 p.m. on October 25, 1994, Employee #1 was bending over, picking a grocery order by hand. A pallet that was directly above the slot where Employee #1 was picking the order fell, striking him in the middle of the back and causing a lumbar contusion. The lower slot of the Unarco Racking System measured 8 ft wide, 43 1/2 in. deep, and 51 in. from the floor to the horizontal beams. Note that the pallet fell between the front and rear horizontal beams.
