Employee injured in fall from elevated forklift

At approximately 6:45 p.m. on January 3, l991, Employee #1, of M.C. Specialties, Inc. (dba Covina Kawasaki) was on an elevated forklift, repairing a roof leak on a single-story office building with a piece of plywood. A coworker was operating the forklift, which was facing the building's north wall. Employee #1 lost his balance and fell approximately 13 ft to the cement floor, suffering cuts and lacerations to his shoulders, elbows, and left knee. A proper basket and safety attachments were not provided, as required.
