EMPLOYEE INJURED IN FALL FROM SCAFFOLD

On August 19, 1997, Employee #1 was working at the mid-level of a scaffold erected around a tunnel input gate. This gate valve was approximately 20 ft wide and 20 ft high. Employee #1 was engaged in routine scheduled maintenance. Employee #1 had ascended to the upper level of the scaffold to retrieve two bundles of zinc anodes. These are zinc rods were about 39-in. long and about 0.75-in. in diameter, and came packaged twelve to a bundle. Each bundle weighed approximately twenty-five pounds. Employee #1 attempted to climb down to his work area while carrying the two bundles of anodes. Employee #1 lost his grip and fell about 18 ft to a concrete deck. Employee #1 rolled on the deck and then fell 20 ft into the gate well.
