Employee suffers multiple fractures in fall with ladder

At about 9:30 a.m. on March 6, 1993, Employee #1, a shop supervisor with Acsco Products, Inc., Burbank, CA, was attempting to remove a portable, ceiling-mounted heating unit. The employee had placed a 20 ft aluminum extension ladder atop a small utility table, apparently to gain greater height because the ladder was too short for the task. He climbed to the desired height and, as he reached to remove the chain that was holding the heating unit, the ladder slipped and fell. Employee #1 fell approximately 12 ft to a concrete floor and sustained multiple fractures.
