Employee's fingertip amputated between cam and cam-follower

At approximately 9:00 a.m., Employee #1 was raising the Maxon RC-3B lift platform of a GMC box truck after unloading five 300 lb blocks of ice. He was holding the cam-follower down when it came in contact with the cam; the tip of his left ring finger became caught between them and was amputated. The cam-follower was supposed to be held down with a pin, but Employee #1 said he was not aware of this requirement. He had not been instructed to use the pin.
