Investigation Summary

Investigation Nr: 170191878
Event: 05/26/1992
Employee burned in gasoline fire

At approximately 12:55 p.m. on May 23, 1992, Employee #1, of Cox Petroleum Transport and a Dyn Corporation employee were transferring unleaded gasoline from a commercial delivery tank truck to a military tank truck. Employee #1 had connected the transfer hose and engaged the power take-off driven pump when he heard a loud "pop." The fuel delivery hose had failed, allowing gasoline liquid and vapors to flow inside the vehicle cab. When the vapor or gasoline came into contact with the hot exhaust stack of the diesel engine, a fire broke out. Employee #1 received burns on his arm. He was hospitalized.
