Employee dies of asphyxia when throat pinned by boom

A 16 ft 9 in. truss boom weighing approximately 800 lb was stacked crossways on a stack of lumber approximately 4 ft 11 in. high by 4 ft wide. Both ends of the boom were hanging over the edge of the pile by 5 to 6 ft. Employee #1 was caught under the heavy end as it fell. The boom connecting end (connected to a model 8A-24 forklift) fell across his jaw, mouth, and throat, pinning him. Employee #1 died of asphyxia. The boom was improperly stacked. Employee #1 was untrained in hazards such as this one. The employer was cited by the federal Labor Department, state Labor, and Cal-OSHA for lack of IIPP.
