Employee killed in fall when falsework towers collapse

At approximately 9:00 a.m. on January 23, 1992, Employee #1 was helping move a 66,867 lb falsework platform built around a bridge pier cap and falsework towers to adjacent towers. The 36 ft wide by 50 ft long platform, constructed of steel I-beams and timber, was located about 50 ft above the ground. Employee #1 had positioned himself near the top of a tower to help land the platform, which was being moved by a 230-ton crane. The pick was made with a boom angle of about 68 degrees and a 70 ft radius. While booming to the right, the crane started to tip, causing the platform to strike against the towers. The towers collapsed and Employee #1 fell to his death.
