Employee injured in fall when ladder slips out

At approximately 9:10 a.m. on December 18, 1990, Employee #1 was at a height of 22 ft on an aluminum extension ladder, painting the inside of a recently constructed warehouse. The bottom of the ladder slipped out Employee #1 fell, suffering injuries that required hospitalization. The ladder was not equipped with skid-resistant safety shoes, and it had been placed on a smooth concrete floor.
