Investigation Summary
Employee #1, a cleanup man, was lowered into a 25 ft deep caisson. He was riding in a cage suspended on a chain attached to a crane hook. The chain had been wired to keep it from slipping off the hooks where it was attached. Employee #1 fell into the caisson, sustaining injuries that required hospitalization.
