Investigation Summary
Employee #1 was cleaning ink out of a well. To clean the well properly, the rollers must be running. The guard in front of the rollers was not in place when Employee #1 was cleaning the presses. He was wearing surgical gloves, and the roller caught the tip of his finger and pulled his hand in. Employee #1 was able to shut off the rollers before his arm was pulled in.
