Employee killed when struck by backing garbage truck

At 12:07 p.m. on July 10, 1990, Employee #1finished directing a garbage truck through the transfer center of a non-hazardous waste dump site and then began directing the next truck. The driver of the first truck decided to go to the restroom before leaving the site and started backing up his truck, unaware of the signalman's location. Employee #1 was struck and killed by the backing vehicle. The truck had a working back-up alarm, but Employee #1, with his back to the truck, apparently did not hear it.
