Investigation Summary

Investigation Nr: 170172589
Event: 01/03/1991
Employee injured when struck by machine while moving it

Employee #1 was hired by a food manufacturing plant to move food processing machines into place for operation. He and his assistant were going to move a food chopping machine approximately 3 feet. This machine weighs about 1200 pounds and was up on 2 by 4 blocks waiting to be placed. They were using a forklift truck to lift and move the machine. The forks were placed under the machine, and Employee #1 was placing a chain around the machine to hold it to the forklift thereby steadying it for the move. While he was placing the chain in the proper position, the machine tipped over and struck his hip, breaking it and pinning him to the floor. He was taken to the hospital by paramedics.
