Employee killed when struck in chest by pallet

On December 22, 1998, Employee #1 and coworkers were preparing an order for a customer and needed a pallet to place the material on in order to facilitate handling. While doing so, Employee #1 took a pair of wire cutters and was getting the top wood from a pallet of a wire lath. He climbed on the first pallet and tried to reach the fourth one, in order to remove the wire straps holding the wooden frame around the material. Three pallets fell. One pallet fell in front of Employee #1, a second one on his legs, and a third pallet fell on his upper body, causing fatal injuries. The pallets were stored in piles of four or five pallets. Being press-strapped they were not perfect squares and they were plumbed if stacked on top of each other.
