Employee dies when struck in head by falling pvc pipe

A coworker was unloading a load of pvc pipe from a 45 ft flatbed trailer. Employee #1 was on the driver's side of the trailer. The tractor was a Freightliner. The forklift was a Clark GCS 15, serial #G127-0089-7645KOF041. A bundle of schedule 40 pvc pipe, which weighed about 1,971 lb, fell on Employee #1, causing fatal head injuries.
