Investigation Summary
A four man team was assigned to install insulation along with a metal rooftop on a prefab metal building measuring 650 ft long by 37 ft high. The team worked in pairs. One pair would roll the insulation in place and the other pair would lay the metal rooftop sheet. The employees laying the metal rooftop sheets would carry the sheets in front of themselves and lay them in place. One of the two installers, the lead man, would be approximately two to four steps ahead of the other. On this day the lead man approached the leading edge and stopped at the designated spot. However, Employee #1, the trailing installer, did not stop. He stepped on the insulation and fell through to the subgrade gravel. Employee #1 died.
