Investigation Summary
Employee #1 was working on an 8:12 pitched roof. Coworkers had just removed old asphalt shingles, which he was throwing to the ground. While he was at the edge of the roof Employee #1 fell approximately 25 feet to a concrete sidewalk. Employee #1, who was not wearing fall protection, was killed.
