Investigation Summary
At approximately 3:50 p.m. on October 26, 1989, Employee #1 was watching a drilling operation and taking samples of material when a pickup truck rolled toward the drill rig. Employee #1 turned and saw the pickup at the same moment it hit him. He was pinned between the drill rig and the front of the pickup. The crew extracted Employee #1 and administered first aid, then transported him approximately 75 miles to the nearest hospital. He died of multiple internal injuries.
