Employee crushed to death by falling steel platform

A vertically-movable steel work platform was being constructed as a component of an automated aircraft maintenance stand. Although the platform did not have the final mechanical stops installed, the platform was being lowered by an employee pushing the "lower" button to allow workers better access to the bottom of the platform. The platform was lowered too far and the screw rods used to raise and lower it screwed out of the nut attached to the platform. Employee #1, who was under the platform, was crushed and killed.
