Investigation Summary

Investigation Nr: 14415459
Event: 03/02/1993
Employee killed when struck by falling bales

Employee #1 was checking bales with a moisture probe and separating dry bales from wet ones. The bales were stacked four high and measured 4 ft by 4 ft by 8 ft. He worked to the last row, and checked the bottom two bales for moisture as before, probing each 4 to 5 times on the long side. As he began to walk away, another employee saw the bales start to fall and yelled. Employee #1 took one step, then two upper bales fell on him, killing him.
