Investigation Summary
Employee #1 was on a roof performing built-up roofing. Two other employees were rolling out a sheet of single ply, modified bitumen. The employee lifted up the roof warning line and began to roll out the single ply as he walked backward. He fell approximately 30 feet to the ground and he died.
