Employee dies after slipping from roof edge

On October 12, 1994, Employee #1 and another employee were preparing to load shingles on the second story roof of a building under construction. They entered the roof from a third story window at 7:00 a.m. when the felt-paper roof covering was still slippery from morning dew. Employee #1 entered the roof and walked adjacent to where a ground-based, vehicle-mounted, motorized conveyor was ready to move packaged shingles to the roof level. Employee #1 was nailing a 2 by 4 at the roof's edge to provide a foot ledge. He apparently slipped and fell 25 feet to the ground. He was pronounced dead at the hospital at 9:37 a.m. the same day.
