Investigation Summary
Three pallets of boxed material were stacked in a warehouse against a wall. Approximately 3 ft away was a stack of plywood. Employee #1 was standing between the plywood and the material when the top two pallets became displaced and pinned Employee #1 against the plywood bundle, killing him.
