The Department of Labor (DOL) has submitted the following public information collection requests (ICRs) to the Office of Management and Budget (OMB) for review and approval in accordance with the Paperwork Reduction Act of 1995 (Pub. L. 104-13, 44 U.S.C. Chapter 35). Description: ETA Form 8650/8559 is a petition used by American workers applying to the U.S. Department of Labor for eligibility to receive workers trade adjustment assistance in accordance with provisions of the Trade Act of 1974, as amended. The petition initiates action on the part of the Department to determine if workers are eligible.

Counts of claims-taking and benefit payment activities are used in budget preparation and control, program planning and evaluation, personnel assignment, actuarial and program research, and for accounting to Congress and the public. This collection is authorized under the Social Security Act, Title III, Section 303(a)(6).
