Weekly Fatality/Catastrophe Report

This table contains the weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. The fatalities listed here include only those that initially appear to be work-related, but exclude fatalities that do not appear to be work-related, such as an apparent heart attack of a sedentary worker. OSHA investigates all work-related fatalities and catastrophes. After OSHA's investigation is complete, these reports will be updated with inspection results and citation information.

10/13/2011 Grove River Mills, Inc., Monroe, GA, 30656 Worker operating a truck's boom was electrocuted when the boom touched a 14,400-volt power line.

10/19/2011 Blackowiak Disposal Inc., Mound, MN, 55364 Worker removing debris from back of trash truck was crushed between truck's cart tipper and the loading dock.
10/19/2011 All-Town, Incorporated, Marshfield, MA, 02050 Worker rotating truck tires was killed when one of the rings holding the tire in place struck him in the head.
10/19/2011 Performance Abatement Service, Berkeley, CA, 94709 Worker in full suit and mask died from a seizure while removing asbestos.
10/20/2011 Mark Dewsnup, General Contractor, Marysville, CA, 95901 Worker was killed after he was run over by a farm tractor.
