Weekly Fatality Reports

This part of the weekly FAT/CAT Report summarizes preliminary information provided to OSHA from employers, which are required to notify OSHA of fatalities or catastrophes (FAT/CAT) in accordance with OSHA standard 29 CFR 1904.39. After all workplace-related fatalities are investigated by OSHA, information from these initial employer notifications is updated if necessary. All incidents are assigned an event number (OSHA-36 Form) by OSHA's local Area Office. Subsequent inspections by OSHA are assigned an inspection number (OSHA-170 Form).
