Section 88.4 - Code of Iowa (2015): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Warehouse storage area, building B (Fab) - Employees were potentially exposed to hazards related to stored materials in warehouse racking. The employer failed to identify the capacities on all warehouse racking. Warehouse racking included but was not limited to the green warehouse racking located in the parts storage areas - no other identification was found. This condition was noted on or about 07-20-16. (b) Tube Mill - Employees were potentially exposed to hazards related to stored materials in warehouse racking. The employer failed to identify the capacities on all warehouse racking. Warehouse racking included but was not limited to the two warehouse racking units located in the parts storage areas - no other identification was found. This condition was noted on or about 07-20-16. Among other methods, a feasible and acceptable method to correct this hazards to follow the recommendations of ANSI standard MH16.1 - 2008, Specifications for the Design, Testing and Utilization of Industrial Steel Storage Racks.
