At noon on May 21, 2011, a crew was removing a section of an underground power line to repair a power outage at an underground installation in a subdivision. The crew discovered that the fault was located under a driveway, so they parked the lines/feeders in the transformers, located and dug up the line on both sides of the driveway. An employee performed a spike test to ensure that the line/feeder was deenergized. The crew then cut the line/feeder and attempted without success to pull a new section using the old line under the driveway. After a string out procedure was performed, they back fed the line that was faulted under the driveway. The crew did not test the line/feeder to ensure it was still deenergized after powering the transformers. The lines were not marked and employees energized the line they were repairing. The employee entered the trench to strip the line to accept the new line being spliced into it and was electrocuted after grabbing it. | ||||||
