Accident Investigation Summary
Summary Nr: 27395.015Event: 09/04/2012Employee Suffers Heart Attack And Dies
At 11:45 a.m. on September 4, 2012, an employee was assisting two coworkers to manually fill a reactor. After assisting the coworkers at the upper plant to manually dump Caprol ET into the reactor to make approximately 20,000 pounds of fatty acids, he notified the facility maintenance manager that he was not feeling well. He then laid down at which point the manager contacted emergency services. He was taken to a nearby hospital where, approximately two-hours later, he was determined to be dead as a result of a heart attack. Caprol ET was a flake material packaged in 50 pound bags and shipped on wooden pallets. Workers used a hoist to deliver the pallets to the reactor building where the bags were manually dumped into the reactor. During this activity, workers were required to wear flame retardant rated full body coveralls, hard hats, safety boots and safety glasses. The incident investigation revealed that the temperature inside the reactor building was at least 91 degrees Fahrenheit with high humidity.
