Accident Investigation Summary
Summary Nr: 25635.015Event: 08/19/2012Two Employees Are Killed And A Third Sustains Burns From A F
At 12:00 p.m. on August 12, 2012, three employees were working for a demolition contractor. They were demolishing a single-story building some 7.6 meters (twent y-five feet) tall. Two of the employees were using a shovel to clean blast mater ial from the top of a deflector, when a third employee used a cutting torch to c ut the bracket that held the deflector to the ceiling. When the employee used th e cutting torch to cut the deflector from the ceiling, a flash fire occurred. OS HA sampling of the blast material in the blast room determined that the material involved was combustible dust. Two employees sustained thermal and possible blu nt force trauma and they were killed. The third employee sustained burns and he was hospitalized.
