Accident Investigation Summary
Summary Nr: 21975.015Event: 07/12/2012Employee Is Electrocuted When Hand Comes In Contact With Liv
At 4:45 p.m. on July 12, 2012, an employee and a coworker were working in an ele vated scissor lift. The employee was trying to lift and maneuver an 8 inch wide, 20 ft long channel steel beam into place. A portion of the steel beam had been supported by the backrest of a forklift truck operated by the president of the c ompany. The employee and the coworker were elevated close to the ceiling. In t he area where the employee and the coworker were working, was a pre-existing bro ken light fixture, which had exposed live electrical wires that ran to a suspend ed electrical metal conduit. The coworker in a separate scissor lift was also li fting and sliding the same 20 ft steel beam as directed by the employee. The em ployee was pulling the steel beam over the suspended electrical metal conduit, w hen his left hand came in contact with pre-existing exposed live wires that ran from a broken light fixture to the suspended electrical metal conduit. The empl oyee came in contact with the exposed live wires and was electrocuted.
