Accident Investigation Summary
Summary Nr: 22636.015Event: 07/05/2012One Employee Killed, Two Injured During Hydraulic Fracturing
At 7:30 a.m. on July 5, 2012, three employees at a hydraulic fracturing site were observing a manifold trailer leak. A 12 inch circular metal end cap separated from the low pressure side of the manifold trailer.Employee #1 was struck by the metal end cap and 80 psi water pressure escaping from the unit, knocked 8 feet into an adjacent pumping unit, suffered blunt force trauma to his head, and was killed. Employee #2, struck by the high pressure water, suffered a contusion to the right arm and a laceration to the head. Employee #3, struck by the high pressure water and knocked to the ground, suffered contusion and bruising to the chest and arm area.
