Accident Investigation Summary
Summary Nr: 15155.015Event: 04/17/2012Employee Falls From Tractor And Is Run Over By The Rear Axle
At 9:30 a.m. on April 17, 2012, an employee was standing on the platform/deck of yard tractor observing coworkers detaching the trailer from the tractor. The employee was operating a yard truck to move a trailer into the company parking lot. During the detaching of the trailer from the yard truck, the employee exited the cabin portion of the yard truck without setting the parking brake. The employee stood on the yard truck platform observing another coworker. When the coworker detached the large trailer from the yard truck, the truck began rolling forward, at that point, the employee lost his balance and fell to the ground. The yard truck continued rolling and rolled over the employee's entire body.The employee sustained multiple blunt force injuries.The employee was transported to the hospital, where he later died from his injuries.
