Investigation Summary
On December 5, 2012, Employee #1 of Three Dots was injured. Employee #1, who works in the employer's Cutting Department, returned to his station after morning break and turned a Blue Streak cutting machine on to resume cutting. However, he was distracted, momentarily looked away, and his left hand struck the cutting blade. The right side of the fleshy tip of his finger was cut away by the blade. Employee #1 was taken to a local clinic by another employee, and from there he was transferred by emergency responders to a local hospital, where he was treated for his injury.
